If you would like for SCI to complete your REAC report, please provide the following information:

• Name, Address, phone numbers, etc., for the Housing Authority
• User ID Application Information (first year submissions only)
• Copy of Annual Financial Statements
• Annual or Audited Financial Statements in Word and Excel Files
• Unit months available and unit months leased for all properties
• Federal ID and DUNS numbers
• Date of certification and officers or staff who certify the information
